A statement, the signing of which by a party confirms that a formal list of documents is an accurate reflection of the search conducted for documents which he has, or previously had. Committal proceedings may follow if a party signs a disclosure statement without an honest belief that it is true.

A comprehensive document prepared by the Chapter 11 debtor or other entity proposing a plan of reorganization explaining the background of the debtor, the bankruptcy case and the risk factors of the proposed plan. It must contain "adequate information" to allow a voting creditor to make an "informed judgment about the plan" (ยง 1125(a)(1), Bankruptcy Code).
